How to refer

1. Submit a request at homeboundvaccination.com or call (623) 266-0021.
2. The patient or their representative completes the online consent and screening form.
3. We verify insurance and call to confirm the visit time (within one business day).
4. A licensed pharmacist visits, vaccinates, and observes for 15 minutes.
