How to Arrange an At-Home Vaccine Visit for a Parent

   Talk with your parent first

   Let them know a pharmacist will visit and which vaccines their doctor has suggested. If you're not sure which vaccines they need, our pharmacist can review their history during the call.

   Gather their cards and medication list

   You'll need their Medicare card (Part B and Part D if they have it), any other insurance cards, and a list of current medications. Photos of the cards are fine.

3. 3

   Submit a request online

   Enter your parent's name and home address, and your own phone number if you want us to call you. Pick a preferred time — it's only a reference; we'll call to confirm.

4. 4

   Complete the consent form

   Right after you submit, you'll get a link to a short consent and health-screening form. If your parent can't complete it, a legal representative can. We can't send a pharmacist until it's done.

   We check coverage and call you

   We verify insurance before the visit. If a vaccine isn't covered, we tell you the price first so there are no surprises.

6. 6

   Make sure someone can let us in

   If your parent lives alone, confirm they can answer the door, or arrange for a neighbor or caregiver to be there. Add gate codes or parking notes in the request.
