At-Home Vaccination Clinics for Assisted Living and Group Homes
We bring adult vaccines to assisted living communities, senior communities, group homes, and caregiver organizations across Maricopa County — so residents don't have to travel.
How facility clinics work
- One-time, seasonal (flu/COVID-19), or recurring clinics
- Licensed, immunization-certified Arizona pharmacists
- Benefits check for each resident before clinic day
- Consent and screening forms completed online ahead of time
- 15-minute observation after every vaccine
- Records shared with residents' providers on request
No service fee for facilities with more than 5 residents. Smaller groups pay the service fee only once per visit, no matter how many residents are vaccinated.
Referring individual residents
Case managers, home-health agencies, and caregivers can request a visit for one person using our standard request form, or call (623) 266-0021. Choose "Senior living facility" or "Healthcare provider" when asked how you heard about us.
